College Housing (Lincoln Campus Traditional Program Only) 

If you plan to live in College housing and will need accommodations because of a disability, contact the Office of Residence Life to complete Section A of the Disability Related Housing Accommodation Request Form. After submitting Section A to the Office of Residence Life, students should request their clinical diagnosing/treating professional to complete and submit Section B of the Disability-Related Housing Accommodation Request Form (available from ODS) to the Office for Disability Services. Students requesting to be considered for the accommodation of an emotional support animal must also submit Sections C and D. Students should request the diagnosing/treating professional to complete and submit Section C to the Office for Disability Services; Section D should be completed by the student and the animal’s veterinarian.

Faculty Letters

 Students should contact ODS to request a Letter of Accommodation at the beginning of each semester (Fall, Spring, Summer). This letter lets your teachers know that you have registered with Disability Services and are eligible to use certain accommodations. The letter does not include diagnostic information, and the teacher will not request that you disclose such information. You are not required to notify faculty that you have a disability, and if you do choose to provide such notification, you are not required to use any of the accommodations described in the letter. However, teachers will not provide any academic accommodations without receiving official notification from ODS. When notifying your teachers about accommodations, it’s helpful to contact them early in the semester to allow enough time to make any necessary changes; however, Letters of Accommodation may be requested at any point in the semester.  Lincoln Campus Traditional Program Use the form below to let ODS know which faculty members you wish to receive notification of your accommodation plan this semester. ABE Program

If your accommodation plan includes taking exams in the Testing Center, contact the appropriate office at least one week in advance of your exam to schedule a testing appointment. Your instructor will be notified of your appointment and asked to submit the exam to the Testing Center.

If your accommodation plan includes using alternate format books (e.g., audiobooks or PDF versions), use the form below to provide the necessary publication information. Alternate format books should be requested as far in advance as possible to help ensure timely arrival of the material.
